New Patient Information and Forms

At Suwanee Family Dentistry, the purpose of the new patient appointment is for us to start to form a relationship that will support your ongoing dental health. The new patient exam is scheduled for one hour and involves a significant amount of time with Dr. Heil or Dr. Starnes. This appointment is scheduled with the doctor, not a hygienist, and is for the purpose of collecting information and determining your dental needs and desires. You will be scheduled for a later appointment with one of our hygienists depending on your needs.

First, you will be seated in the consultation room where you will meet the doctor and discuss any specific concerns you have and your expectations for your dental care. We hope that you will be honest with us at all times about your expectations. If the type of dental care you are looking for is not the type of care that we provide, it is important for us to let you know that.

Next, we will gather the information that we need to properly determine your dental needs. This will likely include a panoramic xray, bitewing xrays, photographs, periodontal charting, and oral cancer screening. Additional xrays or study models may be required in some cases. For our patients with few concerns and simple treatment needs, the doctor will develop a treatment plan at the conclusion of your new patient appointment. The plan will be reviewed with you and any questions you may have are answered. At that time you will be scheduled to return to see a member of our hygiene team for a professional cleaning and/or any periodontal services planned.

For our patients with more complex needs, Dr. Heil and Dr. Starnes will dedicate additional time outside of office hours to develop a comprehensive, detailed plan. In these cases, you will be scheduled to return for a consultation at which time your treatment plan will be reviewed and any questions you may have are answered. At that time you will be scheduled for the first phase of the plan, which may include a professional cleaning or periodontal services with our hygiene team.
